Community Connect is a new Social Prescribing service in North and West Oxfordshire.
What is social prescribing?
Social Prescribing is a way in which people living with long term conditions can get access to a variety of support services they need. Issues like help with getting a job, housing, debt management and social contact. Help with these things are often available through local authorities, charities and local community organisations, but few patients or GP staff know about the full range of support available.
The Social Prescribing team will meet with patients and identify what local support or activity is of interest to them. They might help you look at your chosen activity online, or talk about your worries over a cup of tea.
Community Connect links people with community activities to improve mental and physical wellbeing and reduce loneliness. It all starts with a conversation. You can be referred by your GP or health practitioner, or refer yourself.
