New Patient Registration

We are currently open for new registrations for patients living in the practice catchment area.

Patients can register at Hightown Surgery within the ‘inner boundary’ and existing patients who move within the ‘outer boundary’ may remain registered at the surgery as specified in the map below.

 

In order to register with us please request a new patient registration pack from one of our receptionists. If you have an NHS medical card, please bring it along.

To complete your registration you will be asked to complete the questionnaire inside the pack and return it to the surgery.

Alternatively, you can download the 3 forms below required for registration and bring them into the surgery ready completed.

Please note: When registering as a new patient we will require you to bring 2 forms of identification; one to include a photo and one with address confirmation.

If you are from outside the UK, we can check whether you are eligible for NHS treatment.
